Occupational Injury Management Procedure

Injuries are never planned and occur unexpectedly. Bodine is committed to providing treatment for occupational injuries and illnesses. This policy establishes procedures to be followed in the event an occupational injury occurs. The objectives of this policy are as follows:

  • Establish the responsibilities of the injured employee and company.
  • Establish procedures to provide prompt first aid and medical care for injured employees.
  • Establish injury investigation procedures.
  • Provide instructions for prompt and personal contact with the victim's family.
  • Provide prompt transfer of information to O.S.H.A. officials so that an accurate investigation of the incident will result.
  • Provide instructions for family and victim assistance in handling of insurance and other benefits.

Injury Reporting

All occupational injuries must be reported to your supervisor immediately regardless of severity. The supervisor must immediately notify the Safety Manager if the injury requires medical attention or if the injury resulted from an unsafe act or condition.

Most host employers or clients have injury reporting procedures that require contractors to report injuries to the host employer as well. It is the responsibility of the job supervisor to notify the host employer if an injury occurred.

Upon notification of an occupational injury, Bodine will provide appropriate medical treatment. Treatment may range from on-site first aid to medical attention provided by a licensed medical provider.

Non-work Related Injuries

Personnel with non-work related injuries causing temporary physical limitations or leaves of absence must be evaluated, at the employees expense, and declared fit for duty by a doctor of the company's choosing prior to returning to work. This policy will apply to personnel who have had a leave of absence due to surgery.

Injury Investigation

Personnel involved, either directly or indirectly, in a occupational injury must participate in an investigation. The purpose of the investigation is to determine the cause of the injury and to develop corrective actions to prevent the injury form occurring in the future.

Required participants in the investigation will include the injured person, the lead person or supervisor, crew members, any witnesses to the injury, the Safety Manager and Operations Manager.

Material and data required to conduct an investigation include:

  • Victim statements
  • Witness statements
  • Photographs of the area (if allowed).
  • Diagrams of the area where the injury occurred.
  • Assessment of the condition of any equipment involved.
  • Environmental conditions.
  • Review of past injuries, if any.
  • Review of any past disciplinary actions, if any.

The results of the investigation will be documented and forwarded to the Corporate Safety Manager and President or Managing Member. Corrective actions will be implemented upon approval.

In the event that the injury resulted from negligence or disregard of safety procedures, disciplinary action will be applied to those responsible up to and including termination of employment.

Family Contact

At least one of the following company officials shall be notified immediately of any severe injury, fatality, or vehicle accident:

For serious injuries, emergency services shall be summoned. The employee shall be transported by ambulance to the nearest hospital. In the event that prompt ambulance service is not available Bodine personnel may transport the employee to the hospital. Injured employees shall not drive themselves to the hospital.

One of the company officials shall notify the victim's family as soon as possible using information found on the family notification of medical emergency form in each employee's personnel file. The family shall be notified of the apparent extent of the injury and which hospital the injured employee is being taken to, and offer assistance in arranging transportation to the hospital.

For catastrophic serious injuries involving three or more personnel, the OSHA field office must be notified within eight hours.

In the event of a death at a project site one of the company officials shall notify the victim's family as soon as possible using information found on the family notification of medical emergency form in the employee's personnel file. Notification will be made in person if possible. The Safety Manager will be responsible for immediately contacting the OSHA field office.

Bodine will offer to assist in arranging contacts with physicians, funeral directors, religious support, and transportation.

The Safety Manager will be responsible for preparing the claims for all benefits due to the beneficiary. He should also meet with the next of kin after the funeral to explain insurance and benefits.
